It's not hard really.
1) be in a hurry to get the deposit done
2) quickly mark a pile of 50's & 100's
3) send the deposit to the bank
4) realize you forgot to record the serial #'s on said bills

maybe instead of serial numbers, barcodes can be used, barcode scanners already exist.

You can get stuff to read the serial numbers on bills. The pens that Masterraistlin mentions already exist too, but the key with them is the character recognition software. I'm not willing to pay quite that much for them either, not when I have kids who will enter the numbers for me…
